Transition Planning Support Coaching Package (Designed with "you" in mind!!)

I will review...

  • Present Levels of Performance.
  • Provide Transition Plan Goal Writing Support 
  • Measurable annual transition goals, including benchmarks or short-term objectives.
  • Educational Progress
  • Accommodations and Alternate Assessments.
  • IEP Review Zoom Follow-up (30 minutes)
  • Writing parent concerns and/or request letters
  • Preparation  for IEP Transition meeting 
  • Provide consultation on accommodations and modifications etc.
  • Development of a Parent Input Statement (VISION STATEMENT)
  • Transition planning support will be developed and customized to fit the needs of the client. ( Total up to 3 hours of support)
  •  Determine various Career Pathways, Workforce, Independent living (if desired) and educational support programs in your local area specifically designed for your teen.
